Location: Jackson, TN – Department of Disability and Aging, Madison County. This position is hybrid (in-office and remote) and requires a criminal background check, MVR screening, and review of status on the Department of Health's vulnerable persons registry.
Qualifications
Education and Experience:
- Bachelor’s degree from an accredited college or university.
- One year of professional experience in one or a combination of the following:
- Health care for children, adolescent, or geriatric populations;
- Social work;
- Social or psychological counseling (e.g., mental health, correctional, or behavioral rehabilitation);
- Early intervention and/or service coordination with individuals with disabilities in an educational, social services, or medical setting;
- Teaching early childhood education or assessing individuals with disabilities.
- Additional qualifying graduate coursework in a related field may substitute for experience on a year-for-year basis, up to one year.
Necessary Special Qualifications:
- Possess a valid driver’s license if driving is an essential function of the position.
- Complete a criminal history disclosure form and agree to release all records involving criminal history to the appointing authority.
- Supply a fingerprint sample for a TBI-based criminal history records check.
About the Role
Under general supervision, this position involves professional social counseling and/or service coordination of average difficulty. Employees in this role interview and provide social counseling for clients in programs such as adult protective services, maternal and child health counseling, and individuals with disabilities in educational, social service, or medical settings.
Responsibilities
- Investigate and/or report abuse, neglect, or exploitation of adults and/or children with mental or physical impairments or who are incapable of self-care to determine the need for protective services.
- Interview and assess clients, family members, and other involved parties, including alleged perpetrators, to gather information for determining risk levels and needs for services.
- Implement a service plan that addresses the needs or concerns of the client(s).
- Refer and/or coordinate services for clients to reduce risk and increase autonomy.
- Identify potential risk factors as part of client assessments and monitor the progress of service plans.
- Respond to questions and inquiries from clients, families, caregivers, and others according to departmental policy.
- Document services implemented, client contacts, observations, progress, and relevant information in client files and databases.
- Strategize solutions as a member of a multi-disciplinary team.
- Use appropriate equipment to assess or implement client services.
- Educate government departments, local agencies, law enforcement, the general public, and other external entities about agency programs.

Monthly: $3,631.00 - $4,533.00
Annual: $43,572.00 - $54,396.00
