JOB TITLE Social Care Network CoordinatorDEPARTMENT AdministrationFLSA CLASSIFICATION Non-ExemptREPORTS TO Corporate Compliance OfficerDATE March 2026JOB SUMMARYResponsible for the screening, monitoring, and oversight of all Healthy Alliance Social Care Network activities within the Rescue Mission of Utica. ESSENTIAL JOB FUNCTIONS Conduct screenings and referrals of clients through UniteUs system.Responsible for monitoring and tracking of data related to Healthy Alliance services provided to clientsGenerate and submit client data reports to supervisorCoordinate incoming and outgoing referrals with program staff & supervisorsResponsible for gathering and processing client data and documentationConduct ongoing Healthy Alliance trainings with approved staff and supervisorsStay abreast of all updates and regulatory changes in the Medicaid Redesign Project on both federal and state levels.Build trust and rapport with clients of all Rescue Mission programsAbide by all Mission, program and safety policies, procedures and guidelines.Attend and participate in regular meetings and trainings as requested by your supervisor and Healthy Alliance.All other duties as requested by your supervisor.MINIMUM REQUIREMENTSHigh School Diploma or equivalent.Minimum 2 Years experience in Human Services requiredAssociates degree in related field preferred.NYS Drivers License requiredFamiliarity with database usage and data entry.Experience with health management softwareAdvanced skills with Microsoft Word, Excel, and Outlook.Enthusiastic support of our Mission Statement.Status/Hours: Full time, Monday- Friday 8am-4pm 40 Hours Per WeekPay Rate $25.00-27.00 per hourBenefits: We offer a comprehensive benefit package for eligible employees to include medical, dental, vision, life insurance, 401(k), voluntary benefits, paid time off, holidays, and paid training.The Rescue Mission of Utica is an equal opportunity employer encouraging individuals with disabilities and veterans to apply.
