About the role
New Horizons is seeking a caring and compassionate SOAR Specialist in their Adult Case Management department. The position involves providing comprehensive direct support services to an adult population primarily composed of Serious Mental Illness and dual-diagnosed individuals.
Responsibilities
- Assist with benefit applications, obtain financial information, and advise staff, clients, and families regarding benefit issues.
- Complete interviews with clients to gather information to complete SSI/SSDI applications.
- Ensure all SOAR Medical Summary Reports for client applications are complete.
- Accompany clients to appointments at the Social Security Administration and maintain communication with SSA and Disability Determination (DD).
- Review cases with Local SOAR Lead and attend scheduled staffing meetings with SOAR lead and SEFBHN.
- Complete SOAR medical summary report for individuals' applications using internal/external medical reports, obtain current psychiatric, APRN, or NHTC medical director signatures for summary report.
- Maintain progress of benefit process and provide assistance as required.
- Conduct case management activities for assigned clients, including assessing strengths and needs, service planning, service brokering, service and care monitoring, and client advocacy.
- Communicate essential information regarding client care to all members of the client's treatment team, both within and outside the NHTC continuum of care.
- Provide crisis intervention services as required.
Requirements
- Associates degree from an accredited college or university and 3 years’ experience working with adults who have serious and persistent mental illness.
- Bachelor's degree in a human service-related field with one (1) year of experience working with adults experiencing serious mental illness. Four (4) years experience may be considered in replacement of a degree.
- Certified
- Valid and current Florida Driver's License required with no more than 6 points in the past 5 years.
- Pass Level 2 Background through DCF and AHCA Clearinghouse.
Pay
Negotiable based on experience and qualifications.
