SNF (Skilled Nursing Facility) Rounding MD

About the role

The Southeast Permanente Medical Group (TSPMG) invites applications for full-time and PRN physicians to round on Kaiser Permanente members at contracted Skilled Nursing Facilities (SNFs) located throughout metro Atlanta.

Responsibilities

• Implement appropriate discharge plans to improve patient outcomes
• Leverage AI-powered clinical documentation tools integrated with EPIC, streamlining clinical workflows
• Work with SNF teams to facilitate care coordination
• Participate in shared call rotation

Requirements

• MD or DO Board Certified or Eligible in Internal Medicine, Family Medicine, Hospital Medicine, Geriatrics, or Palliative Care
• Currently licensed or able to obtain license in the State of Georgia
