About the Role
Assesses the ability to independently coordinate tasks associated with the Personal Care Management (PCM) program, assists consumers in developing skills and resources to manage PCA services and activities of daily living.
Responsibilities
- Conduct assessments and home visits
- Coordinate tasks related to the Personal Care Management (PCM) program
- Assist consumers in developing skills and resources for managing PCA services and daily living activities
- Utilize knowledge of community services
- Demonstrate strong oral and written communication skills
- Perform data entry tasks
Requirements
- Written and verbal fluency in English and Spanish is required
- High School Diploma or equivalent is required; Bachelor's degree is preferred
- Valid driver's license and reliable transportation
- Ability to climb multiple flights of stairs

Schedule
