SF Case Manager Lynchburg

About the role

At Moms In Motion, our Service Facilitators are everyday heroes. You’ll spend your days connecting with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and making sure those you serve can live safe, healthy, and independent lives.

Responsibilities

• Travel daily to client homes within the local area.
• Serve as the go-to guide for families navigating waiver programs.
• Write plans of care and assessments that drive meaningful outcomes.
• Troubleshoot challenges such as service authorizations, timesheets, and related tasks.
• Build lasting relationships with families grounded in respect, patience, and trust.
• Work remotely using a laptop or tablet.

Requirements

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required—heart and hands-on experience matter most.
• Tech-savvy enough to use email, documents, spreadsheets, portals, and video calls.
• A valid driver’s license and reliable vehicle (client transport is not required).
• Strong written and spoken communication skills.
• Ability to pass a background check and provide two professional references.

Qualifications

Person-Centered Thinking/Planning training is a bonus.
