About the role

At Moms In Motion, our Service Facilitators are everyday heroes. You’ll spend your days connecting with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and making sure those you serve can live safe, healthy, and independent lives.

Responsibilities

• Travel daily to client homes within the local area.
• Serve as the go-to guide for families navigating waiver programs.
• Write plans of care and assessments that drive meaningful outcomes.
• Troubleshoot service authorizations, timesheets, and related challenges.
• Build lasting, respectful, and trusting relationships with families.
• Work remotely using a laptop or tablet.

Requirements

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; demonstrated heart and experience are valued.
• Proficiency with email, documents, spreadsheets, portal navigation, and video calls.
• A valid driver’s license and reliable vehicle (client transport is not required).
• Strong written and spoken communication skills.
• Ability to pass a background check and provide two professional references.

Qualifications

• Person-Centered Thinking/Planning training is a plus.
