Service Coordinator (Social Worker)

About the role

Westminster Manor in Ft. Walton Beach, FL., is seeking an experienced Service Coordinator/Social Worker for its senior living apartment community.

Responsibilities

• Conduct a person-centered assessment to understand the needs of the residents
• Develop and modify a resident's required services
• Provide advocacy, organization, problem-solving, and results for the elderly and disabled served
• Demonstrate a working knowledge of supportive services and other resources needed for the community's population

Requirements

• Bachelor’s degree in Social Work, Gerontology, Psychology, or Counseling
• Two to three years of experience in social service delivery with senior citizens and non-elderly disabled

Qualifications

• Demonstrated ability to advocate, organize, problem-solve, and provide results for the elderly and disabled served

Skills

• Knowledge of supportive services and other resources needed for the community's population
