Social Worker - Support Service Coordinator
About the role
This role is for a Social Worker to assist patients in Marietta area facilities and homes. The position involves visits in homes and facilities (ALF and ILF's), requiring reliable transportation.
Responsibilities
- Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities).
- Completing Daily Comprehensive Assessments on patients.
- Completing Daily Care Plans on patients.
- Documenting Chronic Care Management time daily.
- Meting Daily Productivity Goals.
- Coordinating discharges at contracted facilities.
- Coordinating Placement.
- Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.).
- Build and maintain professional relationships with referral sources.
- Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.
Requirements
The ideal candidate should be a compassionate Social Worker with excellent customer service skills, and must have reliable transportation due to travel requirements.
Qualifications
A Bachelor’s degree in Social Work or a related field is required. A Master’s degree is preferred. Current licensure as a Social Worker in the state of Georgia is also required.
Skills
Strong interpersonal and communication skills, ability to work independently and as part of a team, and proficiency in Microsoft Office are essential.

Salary-based, full-time, 8-hour shifts Monday-Friday (8am-5pm).
