The Service Coordinator (Mental Health Case Manager) plays a key role in assessing clients, coordinating interdisciplinary team meetings, and ensuring continuity of treatment efforts.

Responsibilities

  • Assess clients to identify individual problem areas necessitating treatment and complete assessments.
  • Coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
  • Implement and oversee the overall treatment plan for clients on the unit, working closely with nursing staff to ensure continuity of care.

Qualifications

  • Two (2) years of full-time experience or part-time equivalent in a mental health setting.
  • A bachelor’s degree with a major in psychology, social work, or behavioral sciences may substitute for the two years of experience.
  • Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
  • Familiarity with regulatory standards.

Pay

The salary range for this position is $24 USD - $27.82 USD per hour. Final salary offered will be based on factors including skills, experience, licensure, and program acuity.

Schedule

On-call: AM & PM shifts.
