Service Coordinator - Early Interventionist (56312)

About the role

This dual role in the Birth to Three Program primarily functions as a Service Coordinator, but may also provide specialized instruction or therapy services and support to families of infants and toddlers with developmental delays or disabilities.

As a Service Coordinator, you will serve as the primary point of contact for families, ensuring access to appropriate services and supports, and facilitating communication among service providers to promote the child's development and well-being. You may also work closely with families, caregivers, and a multidisciplinary team in the evaluation process, create and implement individualized educational plans, and occasionally provide specialized instruction or direct therapy services according to your degree and team need.

Services are delivered in natural environments, such as the child's home or community settings, to support learning in everyday routines and activities.

Responsibilities

• Act as the primary point of contact for families in the Birth to Three Program.
• Ensure families have access to appropriate services and supports.
• Facilitate communication among service providers to promote child development and well-being.
• Collaborate with families, caregivers, and a multidisciplinary team in the evaluation process.
• Create and implement individualized educational plans that promote developmental progress for children from birth to three years old.
• Occasionally provide specialized instruction or direct therapy services based on your degree and team needs.
• Deliver services in natural environments, such as the child’s home or community settings.

Requirements

• At least one year of experience working with families with special needs.
• Degree in early childhood, OT/COTA, PT/PTA, social work, human services, or related field (or equivalent qualifications such as parent facilitator experience).
