About the role

The Service Coordinator manages the delivery of services to regional center clients including those with intensive physical or emotional needs. They ensure that the client's Individual Program Plan and its objectives are pursued. They assist clients in using public and private resources before using regional center funds to purchase services. They monitor vendor services and verify that services are delivered in accordance with the purchase of service authorizations. They evaluate the effectiveness of clients' Individual Program Plans and ensure that appropriate community resources are made available to the client. They conduct annual reassessments of clients' developmental status in conjunction with the interdisciplinary team. Depending on caseload assignments, they may be involved in the Lanterman eligibility determination for children under "Provisional Eligibility."

Responsibilities

  • Manages the delivery of services to regional center clients including those with intensive physical or emotional needs.
  • Affirms the client's Individual Program Plan and its objectives.
  • Aids clients in utilizing public and private resources before using regional center funds to purchase services.
  • Maintains oversight of vendor services and confirms that services are delivered in accordance with purchase of service authorizations.
  • Evaluates the effectiveness of clients' Individual Program Plans.
  • Safeguards appropriate community resources for the client.
  • Conducts annual reassessments of clients' developmental status in collaboration with the interdisciplinary team.
  • May be involved in the Lanterman eligibility determination for children under "Provisional Eligibility."

Requirements

  • A Master's degree in a field related to developmental disabilities.
  • OR a Bachelor's degree in a field that offers content related to developmental disabilities, such as social work, psychology, counseling, special education, or nursing, with three years of employment in a field related to developmental disabilities.

Qualifications

  • Fluency in a language in addition to English is desirable and may be required.
  • Possession of a valid California driver's license, and minimum legal automobile liability insurance required; the ability to provide personal automobile transportation during working hours and after-hours emergencies or access to another form of reliable transportation that meets these minimum legal requirements.

Skills

  • Strong interpersonal and communication skills.
  • Ability to manage multiple tasks and prioritize responsibilities.
  • Knowledge of developmental disabilities and relevant laws and regulations.
  • Proficiency in Microsoft Office Suite.

Pay

$62,244 - $68,652 with opportunities for 5% increases annually.

Schedule

Hybrid work arrangement with a blend of in-office and home-based work may be available for new employees after successfully demonstrating achievement of performance expectations following at least 3 months of in-office work.
