North Bay Regional Center provides advocacy, services, and support to children and adults diagnosed with intellectual and developmental disabilities and their families in Napa, Sonoma, and Solano counties. NBRC is a nonprofit private corporation that contracts with the Department of Developmental Services to coordinate services and support for individuals with developmental disabilities. We serve over 12,000 people living within Napa, Sonoma, and Solano counties.

Note: Bilingual Spanish/English skills are required.

About the Role

Under the general direction of a Case Management Supervisor, this role is responsible for the full range of case management duties, including cases requiring intensive case management and high levels of expertise. The Service Coordinator implements clients’ service plans and, if assigned to support young children (ages 3-6), assists Intake with determining Lanterman eligibility for children under “Provisional Eligibility” criteria prior to turning 5 years old.

Responsibilities

  • Manages the delivery of services to regional center clients, including those with intensive physical or emotional needs.
  • Ensures the client’s Individual Program Plan (IPP) and its objectives are pursued.
  • Assists clients in using public and private resources before utilizing regional center funds to purchase services.
  • Monitors vendor services and verifies that services are delivered in accordance with purchase of service authorizations.
  • Evaluates the effectiveness of clients’ Individual Program Plans.
  • Ensures appropriate community resources are made available to the client.
  • Conducts annual reassessments of clients’ developmental status in conjunction with the interdisciplinary team.
  • May conduct client assessments on an occasional basis.
  • May be involved in Lanterman eligibility determination for children under “Provisional Eligibility.”
  • Acts occasionally, as assigned, in a supervisory or lead capacity (e.g., Officer of the Day or Acting Supervisor).

Requirements

  • Master’s degree in a field related to developmental disabilities.
  • OR Bachelor’s degree in a field related to developmental disabilities (e.g., social work, psychology, counseling, special education, or nursing) and three years of employment in a related field. Minimum education requirement may not be waived.
  • Bilingual: English/Spanish (required).
  • Ability to provide personal automobile transportation during working hours and for after-hours emergencies or access to another form of reliable transportation that meets minimum legal requirements.

All employment at NBRC requires face-to-face, in-person interactions with clients, family members, community, and co-workers. Candidates must live within commute distance to their preferred NBRC office and the geographical location of their assigned clients. While 100% remote work is not available, a hybrid work arrangement (in-office and home-based) may be available after successfully demonstrating performance expectations following at least 3 months of in-office work.
