Responsibilities
- Link residents with supportive services that promote independence, improve quality of life, and help residents remain safely housed.
- Conduct resident assessments, develop service plans, coordinate educational and wellness programs, provide referrals to community resources, and maintain required documentation.
- Collaborate with property management and local service providers while ensuring compliance with HUD regulations and company policies.
- Empower residents to live independently by connecting them with essential services, community resources, healthcare providers, and social service agencies.
- Build and maintain a robust network of local agencies, providers, and wellness resources.
- Educate and guide residents and families through Federal, State, and local programs tailored to senior adults.
- Aid residents in accessing benefits such as Medicare, Medicaid, SNAP, SSI, transportation, and other assistance programs.
- Coordinate wellness and educational programs that promote healthy aging and independent living.
- Conduct regular follow-up with residents to ensure services remain appropriate and effective.
- Maintain accurate resident files and documentation in compliance with HUD and company requirements.
- Complete assessments, referrals, case notes, and reports in a timely manner.
- Respond to resident concerns and assist with problem-solving.
- Create and maintain an up-to-date directory of community resources for residents and staff.
Qualifications
- Bachelor’s degree in Social Work, Gerontology, Psychology, or related field (Required)
- Knowledge of local services, state and federal programs, and senior resources.
- Understanding of Medicare, Medicaid, Social Security and other public benefit programs for seniors.
- Strong case management and problem-solving skills with a team-oriented attitude.
- Excellent communication and relationship-building skills.
- Organized, resourceful, and ready to take initiative.
