Senior Living Occupational Therapist- Home Health

Occupational Therapist, OT

BAYADA Home Health Care is seeking a Full time Occupational Therapist for a position in Vermont. The role involves outpatient home health visits for adult and geriatric patients in Addison County, Vermont.

Responsibilities

• Perform outpatient therapy evaluations in designated senior living communities or group home settings
• Develop and implement a plan of care for patients based on current research-based approaches
• Maximize functional independence for clients through rehabilitative care and skilled nursing
• Collaborate with home health therapy partners to transition care from home health to outpatient services
• Achieve client-centered goals in the client's living environment
• Educate clients, family members, and caregivers on care plans created for clients
• Utilize an outpatient EMR platform to deliver and document individualized treatment approaches

Qualifications

• A current Vermont Occupational Therapist license
• Prior home care experience preferred, but not required
• Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
• One year of recent work experience as an Occupational Therapist
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills, especially with electronic medical records (EMR)

Additional Information

• Base Pay: $45.00-50.00 per hour
• Flexible schedules, no holidays, no weekends, no on-call
• Bonus available for full-time employees
