Senior Living Occupational Therapist- Home Health

Occupational Therapist, OT

BAYADA Home Health Care is seeking an experienced Occupational Therapist, OT, for a Full time opportunity in Vermont. The position involves outpatient home health visits for adult and geriatric patients in senior living communities or group homes.

Responsibilities

• Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group home settings.
• Utilize current research-based approaches to care to maximize functional independence for clients.
• Collaborate with home health therapy partners to transition care from home health to outpatient services.
• Educate clients, family, and community caregivers on care plans to achieve a team approach to maximizing functional independence.
• Utilize outpatient EMR platform to deliver and document individualized treatment approaches.

Qualifications

• A current Vermont Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information

Base Pay, depending upon qualifications and subject to negotiation: $45.00-51.00 per hour.
