Position Summary
The Senior Integrated Health Care Clinician provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care, behavioral health and dental services. The Senior Integrated Health Care Clinician will foster positive working relationships with co-workers, clients and external key stakeholders.
About the role
- Works as part of a multidisciplinary team to efficiently and effectively meet the behavioral, primary, and dental health needs of the client
- Conducts program specific screening and evaluation and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM)
- Develops and documents clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant
- Engages in facilitating group (outpatient or intensive outpatient) as well as seeing patients across the lifespan when necessary
- Communicates in a positive, effective manner with client, family, and all internal and external providers
- Attends and is prepared for scheduled supervision
- Seeks additional supervision or consultation as needed and follows through with supervisory directives
- Interfaces and collaborates with community providers to provide positive system development for clients and families, to promote Wheeler Clinic's reputation as a quality service provider
- Provides culturally competent care and maintains sensitivity to and respect for clients' cultural traditions, values, and beliefs
- Reports child abuse/neglect as mandated, adheres to confidentiality guidelines and ensures that the program meets all necessary licensing, regulatory, and accreditation standards, as related to the program, staff, records, facility, and care of youth
Responsibilities
- Completes clinical documentation in timely manner
- Demonstrates knowledge of psychopathology and primary modalities of psychotherapy, including individual, group and family
- Correctly triages clinical emergencies, including suicide and danger to others
- Uses best practices and evidenced based practice models for psychotherapy where appropriate
- Makes effective use of topic oriented, focused, time-limited group therapy
- Provides or refers to group when required
- Demonstrates knowledge of the use of psychotropic medications
- Meets direct service productivity goals as defined by program
- Documents all client related information in compliance with agency policy
- Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture
Requirements
- Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field or a doctoral degree in Psychology is required
- Credential(s) Required: Must be fully licensed in CT: LPC, LMFT, LCSW, Licensed Psychologist
