Senior Community Coordinator Physical Therapist/Occupational Therapist (55181)

Position Overview

Provide individualized therapy services to aging adults within assigned senior communities
Support, mentor, and collaborate with therapy teams across multiple sites
Serve as a primary contact for facility administrators and community partners
Help strengthen relationships and promote Greenfield Rehab’s mission and values
Work closely with the Regional Director to ensure clinical excellence, team satisfaction, and smooth daily operations

Qualifications

• Licensed Physical Therapist (PT) or Occupational Therapist (OT) in the State of Wisconsin
• Experience in senior care settings (ALF, SNF, Memory Care, or Home Health) strongly preferred
• Strong communication, leadership, and organizational skills
• Able to support multiple communities and foster positive relationships
• Minimum 1 year of experience preferred, though strong candidates with less experience will be considered
