About the role
The Community Care Coordinator serves as the primary liaison with community partners, collaborates with non-competing senior care providers, conducts personalized consultations, manages a caseload, supports hiring and training, and ensures client satisfaction.
Responsibilities
- Serve as the primary liaison with community partners, including Activities Directors, Executive Directors, Resident Committees, and Sales/Marketing teams.
- Collaborate with non-competing senior care providers in the community (PT/OT, hospice, and home health partners) to support comprehensive care.
- Conduct in-depth, personalized consultations and assessments with clients and families to develop tailored care plans that address medical, functional, and lifestyle needs.
- Serve as the main point of contact for clients and families, overseeing all aspects of their care and ensuring a seamless, high-quality experience.
- Provide coverage for client shifts on an emergency basis when scheduled caregivers are unable to work.
- Support hiring decisions and assist in training and mentoring caregivers to meet the high standards of concierge-level service.
- Match clients with caregivers based on skill set, experience, personality, and lifestyle preferences to promote strong, lasting relationships.
- Conduct regular quality assurance visits and check-ins to confirm that care plans are being followed and that clients and families are fully satisfied.
- Anticipate evolving client and family needs and offer proactive solutions and personalized recommendations.
- Act as a dedicated liaison between clients, families, customer service, and scheduling teams to promote clear communication and timely resolution of issues.
- Maintain accurate, thorough, and confidential records for both clients and caregivers.
- Work scheduled hours every other weekend to support urgent needs and demonstrate a commitment to 24/7 client well-being.
- Partner with the leadership team to continuously improve service offerings and maintain our position as a market leader in concierge senior care.
- Participate in process improvement, system implementation, and integration projects as a super user of key cloud-based business systems.
- Plan, promote, and facilitate at least one event per week at the community level to enhance engagement and visibility.
- Support the successful execution of community-planned on-site and off-site events and activities as a value-add partner.
- Visit potential clients and families at local rehab facilities or hospitals to coordinate safe transitions home and initiate home care services.
Qualifications
- 3-5 years of experience in a high-touch, client-facing role, preferably in senior care, private duty, or hospitality.
- CNA/LPN certification strongly preferred.
- Proven experience in care management, managing complex client relationships, and supporting or leading a team of professional caregivers.
- Strong communication, interpersonal, and problem-solving skills, with a high degree of discretion and professionalism.
- Knowledge of ADLs and IADLs for senior care clients.
- Experience working in a private duty/home care service company.
- Exceptional organizational skills and attention to detail.
- Proactive, service-oriented mindset with a genuine passion for providing extraordinary care.
- Proficiency in WellSky or other care management software and Microsoft Office 365.
- Ability to lift up to 50 pounds.
- Valid driver’s license and reliable, professional transportation.
