What You Will Do to Change Lives

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

This position applies their expertise as a mentor in the program and supports the function by contributing their expertise in:

  • Facilitating morning meetings
  • Helping other Case Managers organize and implement member appointments
  • Conducting training sessions related to case management, specifically addressing more complex situations
  • Assisting in daily touch points to ensure staff are meeting service billing expectations

Shifts: 8:30 AM - 5:00 PM
Days: Monday - Friday

Expected Starting Wage Range

$26.01 - $32.15

What You Bring to the Table (Must Have)

  • A Bachelor’s Degree (in social services or human services) and two years of direct care service in Mental Health or community services OR a High School Graduation or G.E.D. equivalent and five (5) years of direct services in Mental Health or community services; an Associates degree may be substituted for two (2) years of experience
  • Proficiency in Case Management: Demonstrated excellence in case management tasks, including meeting or surpassing direct care service billing expectations
  • Case Management Subject Matter Expert: Proven track record and demonstrated excellence as a subject matter expert in case management, documentation, member engagement, outreach, and de-escalation
  • In-depth knowledge and application of best practices in the aforementioned areas
  • Exemplifies support of positive work culture
  • Proven ability to build effective relationships with team members, stakeholders, and clients
  • Demonstrated involvement in training initiatives, with a track record of successfully training others
  • Proactive assistance with onboarding and orientation of new hires
  • Sensitivity to multi-cultural populations and issues is required
  • Must be at least 18 years of age
  • Must be CPR, First Aid, and CPI certified (hands-on course) on date of employment or prior to providing direct client care and maintain current certification throughout employment
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use the company vehicle to drive clients to appointments and groups, etc.
