Mental Health Case Manager

About the role

Provide case management for Renewal reentrants with mental health and/or drug and alcohol diagnosis/histories. Act as the primary contact for reentrants and assist with connecting them to clinical services.

Responsibilities

• Aid reentrants in managing their medication, treatment, and community schedules.
• Create and implement a case management plan for reentrants.
• Ensure compliance with program guidelines and assist the supervisor in work release center operations.
• Initiate and develop policies and procedures for the work release center.
• Maintain accurate records for the work release center.

Requirements

• Knowledge of Criminal Justice, Social Work, Psychology, Counseling, Public Policy, or related fields.
• Human relations skills are essential.
• Familiarity with Criminal Justice/MH systems and community resources.
• Two years of experience providing mental health and substance abuse services.
• Demonstrated ability to work effectively with all levels of management, employees, and reentrants.
• Effective communication and helping reentrants express their needs and aspirations.
• Bachelor's degree in Criminal Justice, Psychology, Counseling, Social Work, with a minimum of two years of related field experience.
