Senior Care Physician
JOB DUTIES
UCLA Health is seeking a dedicated Senior Care Physician to join their innovative Senior Care Model clinic in Encino. This role is designed to deliver high-value, patient-centered primary care for adults aged 65 and older.
Built on the Age-Friendly Health System’s “5Ms” framework—What Matters, Medication, Mentation, Mobility, and Multi-Complexity—this clinic will provide longitudinal primary and acute care services tailored to the needs of the aging population.
You will have the opportunity to:
- Provide comprehensive primary care for a smaller patient panel to enhance patient access and personalized care.
- Enjoy sharing quality visit times with your patients with longer visits.
- Work in a collaborative, interdisciplinary team including Clinical Advisor RNs, Comprehensive Care Coordinators, and Licensed Clinical Social Workers to support patient care.
- Dedicate 32 hours per week to direct patient care, with additional time allocated for administration and chronic care management.
- Develop professionally through specialized education focused on chronic care management, value-based care, and population health strategies.
- Collaborate with the Senior Care Model Medical Director to drive quality improvement initiatives and evaluate new care delivery models.
JOB QUALIFICATIONS
To be successful in this role, you should have:
- A minimum of 5 years of experience in primary care for adults aged 65 and older.
- A current California medical license.
- Experience with geriatric care and chronic disease management.
- Strong interpersonal skills and the ability to build rapport with patients and families.
- Proficiency in electronic health records (EHR).
- Ability to manage a caseload of 10-15 patients per day.
- Experience with population health management and value-based care.
PAY
The target base salary range for this position is $270,000.00 - $300,000.00 annually. Productivity models, Quality Incentives, and additional income channels may supplement base salary for a total compensation of up to $450,000 annually.
