About the role
To positively impact patients and their families, we provide exceptional home health, palliative, and hospice care. Our care teams serve as way finders and way makers by delivering individualized care plans and experiences, wherever needed.
Responsibilities
- Deliver exceptional care to patients and families in their homes, ensuring comfort and dignity.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans.
- Engage in meaningful conversations with patients and families, offering support and companionship.
- Participate in the We Honor Veterans program to offer companionship and care to Veterans and their families.
Requirements
- Active or retired military personnel with a strong understanding of the military culture and code of honor.
- Excellent communication skills to build trust and rapport with patients, families, and caregivers.
- Ability to provide emotional support and share experiences with Veterans and their families.
- Strong interpersonal skills to foster a sense of community and mutual respect.
Qualifications
- Bachelor's degree in a healthcare-related field or equivalent experience.
- Experience working with hospice, palliative, or home health care settings preferred.
- Knowledge of hospice and palliative care principles and practices.
- Valid driver's license and reliable transportation.

- Strong emotional intelligence and empathy.
- Effective communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to adapt to changing situations and environments.
