About the role

To positively impact patients and their families, we provide exceptional home health, palliative, and hospice care. Our care teams serve as way finders and way makers by delivering individualized care plans and experiences, wherever needed.

Responsibilities

  • Deliver exceptional care to patients and families in their homes, ensuring comfort and dignity.
  • Collaborate with interdisciplinary teams to develop and implement personalized care plans.
  • Engage in activities that promote healing and validation for Veterans and their families.
  • Participate in the We Honor Veterans program to offer companionship and support to Veterans and their families.

Requirements

  • Active or retired military personnel with a strong understanding of the military culture and code of honor.
  • Experience in providing compassionate care to individuals in need.
  • Ability to build trust and foster a safe environment for patients and families.
  • Strong communication skills to connect with patients, families, and other healthcare providers.

Qualifications

  • Bachelor's degree in a related field (healthcare, social work, etc.) or equivalent experience.
  • Current certification in CPR and first aid.
  • Experience working in a hospice or home health setting preferred.

Skills

  • Excellent interpersonal and communication skills.
  • Strong emotional intelligence and empathy.
  • Ability to work independently and as part of a team.
  • Knowledge of hospice and palliative care principles and practices.
