Seeking Military Veterans to Serve and Support our Hospice Veterans

About the role

To positively impact patients and their families, we provide exceptional home health, palliative, and hospice care. Our care teams serve as way finders and way makers by delivering individualized care plans and experiences, wherever needed.

Responsibilities

• Deliver exceptional care to patients and families in their homes, ensuring comfort and dignity.
• Collaborate with interdisciplinary teams to develop and implement care plans tailored to each patient's needs.
• Engage in activities that promote healing and validation for veterans and their families, such as listening, reminiscing, and sharing experiences.
• Participate in the We Honor Veterans program to offer companionship and care to veterans and their families.

Qualifications

• Experience in home health, palliative, or hospice care is preferred.
• Strong interpersonal skills and ability to build trust and rapport with patients and families.
• Knowledge of military history and culture is beneficial, but not required.
• Ability to work independently and as part of a team.
