Seeking Military Veterans to Serve and Support our Hospice Veterans

About the role

To positively impact patients and their families, we provide exceptional home health, palliative, and hospice care. Our care teams serve as way finders and way makers by delivering individualized care plans and experiences, wherever needed.

Responsibilities

• Deliver compassionate and personalized care to patients and families in their homes.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Engage in activities that promote healing and validation for Veterans and their families.
• Participate in the We Honor Veterans program to offer companionship and support.

Requirements

• Active or retired military personnel.
• Strong interpersonal skills and ability to build trust and safety with patients and families.
• Experience working with hospice and palliative care patients is preferred but not required.
• Willingness to engage in discussions about military service and shared experiences.

Qualifications

• High school diploma or equivalent.
• Basic understanding of hospice and palliative care principles.
• Ability to work flexible hours, including evenings and weekends.
