About the role

This position is from 7:15 AM – 2:45 PM. The Skilled Nurse performs in accordance with physician orders and the plan of care under the direction and supervision of the RN. Services are furnished in accordance with Agency policies and procedures.

Responsibilities

  • Administer medication in accordance with standards of practice.
  • Encourage social/rehabilitative skills and encourage self-care and independence.
  • Perform patient treatments in accordance with the plan of care.
  • Assist the patient in learning appropriate self-care techniques.
  • Prepare clinical and progress notes to document care provided.
  • Monitor, document accordingly, and report any change in patient condition to the Clinical Director.
  • Promote activities to achieve a safe and healthy environment.
  • Observe aseptic technique when performing patient procedures/care.
  • Practice Standard Precautions at all times.
  • Work with other members of the clinical team to develop and improve the plan of care.
  • Assist the patient in achieving established goals in the plan of care developed by the RN CM.
  • Maintain productivity according to the standards of the agency.
  • Participate in patient care meetings, in-services, orientation, and staff meetings.
  • Understand when to request supervisory assistance.
  • Adhere to the policies and procedures of the agency.
  • Provide service in accordance with agency’s philosophy, standards, policies, and scope of practice.
  • Participate in Performance Improvement and adhere to HIPAA guidelines.
  • Other duties as assigned.

Requirements

  • Licensed nurse in the State of Ohio, in good standing.
  • Current BLS CPR Certification.
  • One year or more experience as an RN/LPN in home health preferred.
  • Must be able to read and understand various documents used by the agency including plan of care, flow charts, patient charts, agency policies, and procedures.
  • Knowledge of Medical Terminology is required.
  • Must be able to use a computer, laptop, or tablet.
  • Requires math skills including ability to add, subtract, multiply, and divide whole numbers.
  • Must practice patient confidentiality to protect the patient’s privacy.
  • Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
  • Must promote customer service and always provide exceptional patient care, doing whatever we can to please our patients while staying within the rules of the agency and each program served.
  • Type and computer skills are required for this position.
