About the Role
We are seeking a talented, passionate Skilled Nurse to perform in accordance with physician orders and the plan of care under the direction and supervision of the RN. This position operates on a schedule from 6:30 AM to 4:00 PM and supports patients in their homes, helping them live happier and healthier lives.
Responsibilities
- Administer medication in accordance with standards of practice.
- Encourage social/rehabilitative skills and promote self-care and independence.
- Perform patient treatments in accordance with the plan of care.
- Assist the patient in learning appropriate self-care techniques.
- Prepare clinical and progress notes to document care provided.
- Monitor, document, and report any change in patient condition to the Clinical Director.
- Promote activities to achieve a safe and healthy environment.
- Observe aseptic technique when performing patient procedures/care.
- Practice Standard Precautions at all times.
- Work with other members of the clinical team to develop and improve the plan of care.
- Assist the patient in achieving established goals in the plan of care developed by the RN CM.
- Maintain productivity according to the standards of the agency.
- Participate in patient care meetings, in-services, orientation, and staff meetings.
- Understand when to request supervisory assistance.
- Adhere to the policies and procedures of the agency.
- Provide service in accordance with agency’s philosophy, standards, policies, and scope of practice.
- Participate in Performance Improvement and adhere to HIPAA guidelines.
- Other duties as assigned.
Requirements
- Licensed nurse in the State of Ohio, in good standing.
- Current BLS CPR Certification.
- One year or more experience as an RN/LPN in home health preferred.
- Must be able to read and understand various documents used by the agency including plan of care, flow charts, patient charts, agency policies, and procedures.
- Knowledge of Medical Terminology is required.
- Must be able to use a computer, laptop, or tablet.
- Requires math skills including ability to add, subtract, multiply, and divide whole numbers.
- Must practice patient confidentiality to protect the patient’s privacy.
- Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
- Must promote customer service and provide exceptional patient care.
- Type and computer skills are required for this position.
