Ryan White Case Manager, School of Medicine

The Ryan White Case Manager unit is part of the VCU School of Medicine, focusing on improving access to and retention of medical and support services for individuals living with HIV.

Duties & Responsibilities

• Responsibility 1 (70%): Case Management - Screen for initial and annual eligibility for the Ryan White grant program, perform assessments, identify service needs, refer to support services, and manage care plans. Complete psychosocial assessments and documentation, respond to referrals, and ensure patient eligibility for grant-funded services.
• Responsibility 2 (15%): Data Entry - Collect and enter CAREWare data in accordance with policies and procedures, ensuring accuracy in EPIC, the electronic medical record.
• Responsibility 3 (5%): Quality Management - Participate in Quality Management and Quality Improvement activities as required.
• Responsibility 4 (7%): Other Duties as Assigned - Attend supervisor-required meetings, trainings, and competencies, attend one community event annually.
• Responsibility 5 (3%): Professional Development/Continuing Education - Attend or complete at least one pre-approved professional development/continuing education activity through the VCU Ryan White Programs.

Qualifications

• Minimum Qualifications: 3 years of Case Management experience in a healthcare delivery system, knowledge of Medicare, Medicaid, and other state or federally funded assistance programs, HIPAA compliance, excellent customer service and interpersonal skills, strong time management and organizational abilities, superior teamwork behaviors, proficiency in MS Office and data entry programs, sensitivity to confidential health information and HIV/AIDS issues, outstanding communication skills, and a commitment to fostering an inclusive environment.
• Preferred Qualifications: Knowledge and experience with the Ryan White grant program.

Salary Range

$45,000-65,000 (Commensurate with experience)

This is a restricted position with no set end date and continued employment is dependent upon project need, availability of funding, and performance.
