Position Summary

The purpose of this position is to: 1) implement grant goals, outcomes, and activities, including evaluation, that are related to chronic disease, including diabetes, hypertension, and cardiovascular health; and, 2) support training diabetes coalitions in the Appalachian Region as they implement community-based programs for diabetes and chronic-disease self-management.

Duties / Position Accountabilities

  • Assist in the planning, coordination, implementation, and evaluation of grant and program activities to meet outcomes.
  • Conduct data collection and analysis to measure program impact and effectiveness using mixed-methods quantitative and qualitative evaluation methods.
  • Attend conferences for training, networking, and professional development to stay up to date on the field of chronic disease, health promotion, and disease prevention.
  • Participate in the planning and implementation of community-based activities, meetings, conferences, trainings, etc.
  • Develop clear and thorough evaluation summaries of findings to present in long-form documents and short-form presentations to best communicate program successes and challenges.
  • Provide technical support to key stakeholders and partners to meet grant goals.
  • Use Microsoft Teams to virtually collaborate with teams and assist in the scheduling, agendas, and meeting minutes.
  • Efficiently and effectively coordinate communication among team members and with community partners and various stakeholders.
  • Lead training, implementation, technical assistance and evaluation of evidence-based chronic disease prevention and management programs (i.e., National DPP, DSMES, and CDSMP).
  • Assist with managing social media communications.
