RN Health Care Coordinator

GENERAL SCOPE

Maintains the standards of nursing care and practice in the Day Services Department. In accordance with Federal, State and local standards, guidelines and regulations.

Responsibilities

• Assists in the development of procedures for the Adult Day program, concentrating on the delivery of the health care services, in conformance with the NYSDOH regulations and consistent with JHR’s and the Adult Day standards.
• Maintains infection control procedures as developed by the JHR. Universal precautions apply to this position.
• Assists in the hiring, training, evaluation and disciplinary actions as appropriate for subordinates as assigned by the Director of Day Services.
• Maintains constant interventions and interaction with nursing staff for the purpose of evaluating the care provided and the development of the staff's capability to render care in changing circumstances.
• Monitors the activities of nursing personnel to insure adherence to standards and regulations.
• Plans for optimal utilization of nursing staff for the delivery of nursing services to meet registrant care needs and advises the Director of changes needed in the staffing patterns.
• Maintains own current knowledge of nursing care requirements for all registrants in the program and assists in the assessments for admissions and discharge planning for the registrants as necessary.
• Works closely with own staff and peers in developing improved nursing care programs and nursing practices for the registrants and, where appropriate, makes recommendations for change to the Director of Day Services.

Qualifications

• Current New York State Registered Professional Nursing license.
• Supervisory experience.

Experience

• Three (3) years clinical experience in an adult day care setting.
• Acute or long-term nursing care is acceptable.
• Previous leadership responsibilities preferred.

Pay

Salary Range: $78,000-$85,000 (Pay information will vary depending upon relevant experience for the position)

Schedule

FT 40 hours Monday-Friday 8:00am-4:30pm, occasional until 6pm
