RN Wound Care

About the Role

The RN Wound Care provides quality patient and family centered care as a member of the health care team, applying standards for professional nursing practice and focusing clinical care to enhance the experience for patients, families, and others. This position oversees the work of other caregivers providing specific care to assigned patients throughout the shift, as required under the scope of the RN licensure.

Responsibilities

• Maintains and demonstrates in-depth knowledge of nursing principles, practices, standards, and techniques in accordance with St. Charles Health System’s policies and Oregon state laws.
• Provides wound and ostomy care for patients per direction of the certified wound and ostomy nurse, including:
  • Wound care assessment
  • Implementation of wound care interventions
  • Communication and collaboration with the care team
  • Support for organizational education activities promoting wound healing and preventing skin breakdown
• Assesses patient conditions and needs using critical thinking and clinical judgment; sets outcomes, implements nursing actions, and collaborates with other disciplines to meet physical, emotional, spiritual, social, and intellectual needs.
• Provides and maintains a safe environment for caregivers, patients, and guests.
• Demonstrates awareness of and supports departmental and organizational goals through participation in continuous quality improvement and departmental activities.
• Participates in activities that promote professional growth and development of self and others.
• Communicates effectively (written and orally) and maintains professional relationships with caregivers, medical staff, patients, and guests.
• Identifies work-related problems and implements solutions within the scope of practice.
• Manages the patient’s experience by clarifying, integrating, and coordinating the roles of the interdisciplinary team.
• Performs standby call as assigned (varies by department).
• Supports the vision, mission, and values of the organization, including Lean principles of continuous improvement.
• Conducts all activities with the highest standards of professionalism and confidentiality, complying with applicable laws, regulations, policies, and procedures.
• Delivers customer service and patient care in a manner that promotes goodwill, timeliness, efficiency, and accuracy.
• Participates in creating a healing environment that supports the wholeness of each individual, patient, and caregiver.
• Provides therapeutic presence by responding to patient needs in a caring way, including active listening, effective communication, and appropriate touch.
• Calculates dosages and administers medications accurately in accordance with organizational policies and procedures.

Additional Duties for Certified Roles

• RN Certified Wound Care: Responsible for wound care and oversight for patients referred to the Wound & Ostomy department, including assessments, interventions, physician consultations, care team collaboration, and educational support for wound healing and skin breakdown prevention.
• RN Certified Wound Ostomy: Responsible for wound and ostomy care and oversight, including wound assessments, interventions, ostomy consultations, stoma site markings, physician consultations, care team collaboration, and educational support for wound healing and ostomy care.

Requirements

• Current valid Oregon RN license.
• Valid Oregon driver’s license and ability to meet St. Charles Health System driving requirements.
• AHA Basic Life Support for Healthcare Provider certification.
• One (1) year of experience in an acute wound care setting.

Qualifications

• Required: Education as required by RN licensure.
• Preferred: Bachelor’s degree in Nursing.
• Preferred Certification: Board Certified Wound Care or Ostomy Care Certification through ABWM or WOCNCB.
