Position Responsibilities
- Conduct admission-level reviews of newly hospitalized patients to support medication reconciliation processes, ensuring accuracy of documented information.
- Identify and escalate potential safety concerns, documentation gaps, or coordination needs according to organizational standards.
- Perform ongoing clinical and discharge readiness reviews to identify barriers to transition, including social, logistical, or system-related factors.
- Collaborate with interdisciplinary teams to support discharge planning and assess eligibility for programs such as Hospital at Home.
- Facilitate communication and coordination but does not replace bedside clinical decision-making.
- Support discharge preparation activities by reviewing documentation for completeness, reinforcing patient and caregiver education related to medications, equipment, and follow-up care, and ensuring alignment across care teams for safe transitions.
- Maintain compliance with HIPAA, confidentiality, and regulatory standards.
- Participate as an active member of the care coordination team and maintain competency in required virtual care technologies.
Position Qualifications
- Required Experience: Minimum of 3 years of recent acute care nursing experience, required.
- Required Education: Excellent communication skills, assessment skills, and clinical judgement required. Graduate of an accredited nursing program, required. BSN preferred.
- Training / Certification / Licensure: Current State of New Jersey nursing license to practice as a registered nurse, required. Current CPR certification BLS, required. Valid driver’s license required. Must be at least 21 years of age and meet insurance requirements. Relevant certification preferred.
Pay
$44.61 - $61.97 Hourly
