Triage RN Position Overview
The Triage Registered Nurse responds to overnight and weekend calls regarding the changing needs of hospice patients. They provide guidance, assess patient needs, and coordinate care via phone conferences. This role requires an experienced nurse with a focus on palliative care.
Patient Care
- Answers calls related to patient care and provides appropriate interventions based on nursing knowledge.
- Evaluates available options for interventions and selects the best course of action.
- Determines the need for a nursing visit and dispatches a visit RN or LPN as necessary.
- Completes and maintains accurate clinical notes regarding patient's condition and care provided.
- Communicates with physicians regarding patient needs and obtains/receives physician orders as required.
- Facilitates the patient's efforts toward self-sufficiency and optional comfort care.
- Counsels and collaborates with the family/caregiver and hospice interdisciplinary group to meet emotional needs.
- Maintains compliance with HIPAA regulations and ensures confidentiality of patient information.
Qualifications
- Current Registered Nurse (RN) license in state(s) of practice.
- Graduate of a professional school of nursing and licensed, certified, or registered in the state(s) of practice.
- Experience in hospice/home care (preferred).
- Physical ability to push/pull, lift/carry up to 30 pounds occasionally, and adapt to various work conditions.
Requirements
- Valid State Driver’s License, automobile in good working order, and automobile insurance in accordance with state and organizational requirements.
- Ability to pass background check, consumer report, and drug test.
Benefits
The position offers a $420 per diem shift wage, with hourly rates of $30/hr for orientation, meetings, and education. Total compensation may include medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to plan terms and eligibility.
