Per Diem RN Triage

Position Overview

The Per Diem Triage Registered Nurse responds to overnight and weekend calls regarding the changing needs of hospice patients. They assess patient needs, provide guidance, and coordinate care via phone conferences. This role requires experience in nursing, particularly in palliative care.

Essential Functions And Skills

• Performs all duties in accordance with the Nurse Practice Act and professional nursing guidelines.
• Answers calls related to patient care and provides appropriate interventions based on nursing knowledge.
• Evaluates available options for interventions and selects the best course of action.
• Determines the need for a nursing visit and dispatches a visit RN or LPN as necessary.
• Completes, submits, and maintains accurate clinical notes regarding patient’s condition and care given.
• Communicates with physicians regarding patient needs and obtains/receives physician orders as required.
• Facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
• Counsels and works in cooperation with the family/caregiver and hospice interdisciplinary group to meet emotional needs.
• Maintains compliance with HIPAA regulations and ensures confidentiality of patient information.

Qualifications

• Current Registered Nurse (RN) license in state(s) of practice.
• Possess or obtain an RN license for the 10 states we serve and/or a compact license to cover the states before hired.
• Graduate of a professional school of nursing and currently licensed, certified, or registered in the state(s) of practice.
• Ability to pass background, consumer report, and drug tests.
• If Driving: Valid State Driver’s License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.
• 1+ years of recent experience in hospice/home care preferred.
• 2+ years of Nursing experience, preferred.
• Physical requirements include pushing/pulling and lifting/carrying up to 30 pounds occasionally.

Requirements/Qualifications

• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their RN license in their primary state of residence (PSOR).
• Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.
• Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.
• Ability to pass DHS background, consumer report, and drug test.
• If Driving: Valid State Driver’s License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.
• Preferred 1+ years of recent experience in hospice/home care preferred.
• 2+ years of Nursing experience, preferred.

The Per Diem rate for this role is $420/day and $35/hour for training/education.
