RN Telephone Triage

About the Role

As our RN Telephone Triage, you will work as a telephone triage Registered Nurse in the Call Center, managing calls from diverse clients including community members, physician practices, managed care members, and Home Care patients. You will perform health assessments using computerized guidelines, provide general health information from approved references, and as needed, provide physician and community service referrals. You will expertly function in a multi-task environment to support various client contracts.

Responsibilities

• Answer incoming phone calls for all product lines and provide thorough and accurate telephone triage.
• Assess patient's physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records, and clinical data.
• Evaluate patient's response and intervene to ensure optimal patient outcome.
• Document all calls according to call center guidelines.
• Promote, advocate, and collaborate to protect the health and safety and rights of each patient.
• Provide patient/family education through assessment of patient/family learning readiness, needs, and ability.
• Recognize the patient or designee as a full partner in providing compassionate and coordinated care based on respect for the patient's preferences, values, and needs.

Requirements

• Required Associate's Degree in Nursing or by endorsement.
• Three (3) years recent hospital, home health, or clinic experience.
• RN License; Current state nursing licenses or valid RN license from a participating state in the National Licensure Compact.
• Current Basic Life Support (BLS) certification issued by the American Heart Association.

Preferred Bachelor's Degree in Nursing.

Pay

$34.14 - $61.23 per hour.
