RN Team Leader (Supervisor) Hospice (Full-Time/Days)

Responsibilities

• Provide end-of-life care to patients and support their families
• Collaborate with interdisciplinary teams to develop and implement care plans
• Administer medications and treatments as prescribed
• Document patient care activities accurately and timely
• Maintain a safe and clean environment for patients and staff

Requirements

• Bachelor's degree in a healthcare-related field
• Current certification in Hospice and Palliative Care (CHPC)
• Experience in hospice or palliative care preferred
• Strong communication and interpersonal skills
• Ability to work flexible hours including evenings and weekends

Qualifications

• Valid driver's license and reliable transportation
• Pass a background check and drug screening

Skills

• Excellent organizational and time management skills
• Proficiency in Microsoft Office Suite
• Ability to handle sensitive and confidential information
