What You’ll Do
- Care Needs & Planning: Implement & revise care plans, monitor health conditions, supplement direct care needs, respond to emergencies or changes in status
- Staff Oversight & Scheduling: Observe care assignments, manage coverage needs, evaluate staff performance, educate & lead staff
- Family & Team Liaison: Communicate changes in resident conditions to families/providers, collaborate with interdisciplinary team
- Documentation: Review and investigate incidents, transcribe orders, audit medical charges, maintain record compliance for accuracy
- Family, Resident, Staff Education: Explain health status, changes, and expectations for care, guide and direct care skills with intervention as needed
Requirements
- Prior Long Term Care Experience
Qualifications
- Licensed to work within the State of Delaware as a RN
- Able to pass background check, drug screen, professional references check & physical evaluation
- Excelling in leadership and clinical competency
- Looking for growth & career development
