About the Role

Generations Hospice is seeking a passionate, dedicated Registered Nurse to join the On‑Call team. You will provide 1:1 hospice care with lower patient‑to‑nurse ratios, collaborate with a strong interdisciplinary team—including social work, chaplaincy, aides, and providers—and receive support from a dedicated internal after‑hours triage team. The position is based at 1028 S. Main Street Suite C, Fond du Lac, WI 54935, with travel throughout the assigned territory.

Responsibilities

Collaborate with the Remote Triage On‑Call Team to deliver hands‑on care to hospice patients and support families during after‑hours.
Assess and manage urgent symptoms quickly and compassionately.
Administer medication or adjust care according to hospice protocols or provider orders.
Communicate with physicians regarding changes in condition and obtain necessary orders.
Offer guidance and support to patients and families to help them understand the current condition and anticipate next steps.
If a death occurs during on‑call hours, pronounce death, provide family support, contact the funeral home, and notify the hospice team.
Complete timely documentation and communication in the Homecare Homebase (HCHB) EMR.

Qualifications

Graduate of an accredited nursing school with current licensure to practice in Wisconsin.
Minimum one year of nursing experience (three or more years preferred).
Experience in hospice or a similar setting preferred.
Knowledge of the hospice philosophy of care.
Commitment to clinical and documentation excellence.
Must be a licensed driver with an insured automobile that meets state and organizational requirements and is in good working order.
