RN / Registered Nurse On Call - Hospice - $5,000 SIGN ON BONUS

About the role

Generations Hospice is seeking a passionate, dedicated Registered Nurse to join the On‑Call team, providing end‑of‑life care during off‑hours. The role offers lower patient‑to‑nurse ratios, strong interdisciplinary support, and a dedicated internal after‑hours triage team.

Office Location: 1028 S. Main Street Suite C, Fond du Lac, WI 54935. Coverage area requires travel throughout the territory.

Schedule

Full‑time 7 ON / 7 OFF schedule. Weekdays Monday‑Thursday 4:30 p.m. – 8 a.m.; Friday starting at 4:30 p.m. through Monday 8 a.m.

Responsibilities

• Collaborate with the Remote Triage On‑Call Team to deliver hands‑on hospice care and support families during after‑hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care according to hospice protocols or provider orders.
• Communicate with physicians regarding changes in condition and obtain orders as necessary.
• Offer guidance and support to patients and families in understanding the current condition and anticipating next steps.
• If a death occurs during on‑call hours, pronounce death, provide family support, contact the funeral home, and notify the hospice team.
• Complete timely documentation and communication.

Qualifications

• Graduate of an accredited nursing school with current licensure in the state of operation.
• Minimum one year of nursing experience (three or more years preferred).
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Valid driver’s license and an insured automobile meeting state/organization requirements.

Skills

• Ability to assess and manage urgent symptoms.
• Proficiency in hospice medication administration and protocol adherence.
• Effective communication with physicians, patients, and families.
• Strong documentation skills using Homecare Homebase (HCHB) EMR.
• Independent decision‑making while collaborating with interdisciplinary team.

Pay

$5,000 sign‑on bonus for external/new hires.
