About the role

Generations Hospice is seeking a passionate, dedicated Registered Nurse for an on‑call hospice position. The role offers lower patient‑to‑nurse ratios for meaningful, unrushed care, strong interdisciplinary team support with Social Work, Chaplaincy, Aides, and providers, and leadership that understands hospice nursing. A dedicated internal after‑hours triage team and Homecare Homebase (HCHB) EMR provide streamlined documentation and support outside regular business hours. The position is based at 1028 S. Main Street Suite C, Fond du Lac, WI 54935, with travel throughout the territory required.

Responsibilities

Collaborate with the Remote Triage On‑Call Team to deliver hands‑on care to hospice patients and support families during after‑hours.
Assess and manage urgent symptoms quickly and compassionately.
Administer medication or adjust care according to hospice protocols or provider orders.
Communicate with physicians regarding changes in condition and obtain orders as necessary.
Offer guidance and support to patients and families in understanding the current condition and anticipating next steps.
If a death occurs during on‑call hours, pronounce death, provide family support, contact the funeral home, and notify the hospice team.
Complete timely documentation and communication.

Qualifications

Graduate of an accredited nursing school with current licensure to practice in Wisconsin.
At least one year of nursing experience (three or more years preferred).
Experience in hospice or a similar setting preferred.
Knowledge of the hospice philosophy of care.
Commitment to clinical and documentation excellence.
Must be a licensed driver with an insured automobile that meets state and organizational requirements and is in good working order.
