Responsibilities

• Assess patient needs and develop individualized care plans
• Implement and evaluate treatment plans in collaboration with the interdisciplinary team
• Provide direct patient care, including medication administration, wound care, and rehabilitation therapies
• Maintain accurate and timely records to ensure continuity of care
• Monitor and report patient progress and concerns to the healthcare team
• Participate in quality improvement initiatives to optimize patient outcomes

Requirements

• Current RN licensure in Pennsylvania
• Minimum 0 years of experience as a Registered Nurse (new graduates welcome to apply!)
• Strong clinical skills and knowledge
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
