About the Role

We are seeking a dedicated and compassionate RN Registered Nurse to join our team at Maplewood Nursing and Rehabilitation, a leading provider of short-term post-acute rehabilitation and long-term skilled nursing care in Philadelphia, Pennsylvania.

Responsibilities

• Provide high-quality, resident-centered care to patients with diverse needs and conditions
• Assess, plan, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team
• Administer medications, treatments, and therapies as prescribed by physicians and as part of our comprehensive care plans
• Develop and maintain effective relationships with patients, families, and caregivers to ensure optimal care and satisfaction
• Participate in quality improvement initiatives to promote positive patient outcomes and liaise with other healthcare professionals to ensure smooth transitions of care
