RN Registered Nurse Internal Medicine FT Days Norwell
Facility: 143 Longwater Drive, Norwell, MA 02061 – SMC Internal Medicine PB. Full-time, 40 budgeted hours per week on a day/evening rotation.
About the Role
Manages individualized, outcome-oriented nursing care through the nursing process. Provides care, treatment, and services by coordinating assessment, planning, intervention, cross-continuum care, and outcome evaluation in accordance with system, departmental, and ambulatory policies. Delivers clinically-based, risk-reducing care and coordination that facilitates cost-effective, quality healthcare across the continuum. Interfaces directly with physicians, care managers, healthcare teams, patients, and caregivers to achieve desired patient outcomes.
Excellent communication skills (written and verbal), a commitment to coaching patients on health-promoting behaviors, and the ability to assimilate complex clinical information are required. Works both autonomously and in teams. Participates in quality improvement, research, and evaluation processes related to patient care management.
Responsibilities
- Medication Administration
- Demonstrates knowledge of and follows SSH policies and procedures for administering and documenting medications.
- Completes medication reconciliation process following SSH policy and procedure.
- Demonstrates safe procedures for the administration of medication and vaccines.
- Plan of Care / Documentation / Patient & Family-Centered Care / Patient Experience
- Develops, discusses, and communicates a prioritized problem and plan of care for each patient.
- Develops, evaluates, and updates individualized plans for patients, documenting outcomes.
- Initiates patient outreach, identifies and documents patient/family/significant other needs, and makes appropriate interventions, evaluating outcomes.
- Continues assessment/reassessment and identifies care needs within established nursing practice.
- Documents all patient care following department of nursing policy, department-based standards, disease processes (CHF, PNA vaccines), and nurse-sensitive indicators (falls, safety risk, isolation, etc.).
- Assesses/reassesses and documents patient's response to each intervention (pharmacological and non-pharmacological) per South Shore Health and ambulatory policies.
- Works on behalf of patient/family, seeking help to represent them when they are unable to represent themselves.
- Raises ethical questions and concerns with the clinical team, seeking resources to help formulate and understand ethical decisions.
- Actively communicates and collaborates with health team members: primary care, specialty care, and ancillary providers and departments.
- Facilitates care across the continuum to ensure patient needs are met in the right setting and at the right time.
- Safety / Quality

- Verifies patient identification with two identifiers prior to any procedure, including "time out," administration of care, medications, labeled specimens, and documents confirming the correct patient, procedure, site, equipment, and consent.
- Complies with current CDC hand hygiene guidelines through proper handwashing, adheres to universal precautions, makes appropriate use of personal protective equipment, and disposes of hazardous materials properly.
- Ensures the environment meets regulatory requirements at all times.
- Understands and demonstrates individual roles and responsibilities in the event of ambulatory codes/emergency preparedness.

- Obtains at least 5 contact hours per year in area of practice.

Requirements
- Graduate of an accredited School of Nursing; BSN preferred.
- Previous ambulatory care RN experience preferred.
- Current RN license (Board of Registration in Nursing, Massachusetts).
- BLS (Basic Life Support) certification required.
- Demonstrated proficiency in ambulatory care nursing, knowledge, and skills.
- One year of telephone triage experience required.
- Excellent verbal and written communication skills.
- Demonstrates flexibility and ability to adapt to changing priorities and regulations.
- Basic computer skills required.
- Demonstrated skills in communication (verbal and written), interdisciplinary collaboration, creative problem-solving, and critical thinking.
- Knowledge of health system and community resources preferred.
Pay
Pay range: $35.01 - $50.00 per hour.
Schedule
- Monday, Tuesday, Wednesday, Friday: 9:30 AM – 6:00 PM
- Thursday: 11:30 AM – 8:00 PM
- One weekend shift per month
- One holiday per year
- Two flu clinics every fall
