RN Registered Nurse Home Health Case Manager

About the role

BJC Home Care is seeking a home health registered nurse to provide 1:1 patient care in an under-served community. The role offers a flexible 40-hour workweek with weekend rotations and paid on-call nights. Candidates must be willing to cover shifts in all St. Louis regions during on-call periods.

Responsibilities

• Evaluate clients and provide specialized nursing services, including preventive and rehabilitative procedures.
• Develop and implement individualized care plans with defined goals, collaborating with the interprofessional team and patient/family/caregiver.
• Promote respect, equity, and empathy in interactions with diverse and vulnerable populations.
• Collaborate with the interprofessional team to ensure safe and timely implementation of care plans.
• Evaluate current nursing care to ensure evidence-based practices and quality patient outcomes.

Requirements

• Valid Driver’s License
• RN License
• Education: Nursing Diploma/Associate's preferred, Bachelor's Degree in Nursing/Home Health
• Experience: 2-5 years of relevant experience

Pay

Salary commensurate with experience.

Schedule

40 hour work week, shift flexible. Weekend rotation: generally every 4th. Paid on-call nights: 4–6 per month. 2 holidays per year.

Benefits

• Mileage Reimbursement at IRS rate .70/mile.
• Up to $15,000 bonus for eligible candidates.
