- Checks for safety and infection control
- Identifies community resource needs
Responsibilities
- Reviews patient history and physical diagnostics
- Reports abnormal results to the physician
- Implements care plans through direct patient care
- Coordinates and supervises healthcare team activities
- Provides skilled nursing care
- Administers preventative and rehabilitative procedures
- Prescribes treatments in various home situations
Skills and Abilities
- Delegates tasks to appropriate personnel
- Uses motivational interviewing and health coaching techniques
- Engages stakeholders in care management
- Understands and applies home care regulations and third-party reimbursement
- Has current IV therapy skills
Education and Experience
- Associate's degree in Nursing [Required]
- Bachelor's degree in Nursing [Preferred]
- 1+ years of relevant clinical nursing experience [Required]
- Recent experience as a case-manager in a Medicare-certified home health agency [Preferred]
Knowledge and Certification
- Registered Nurse (RN) [Required]
- Driver's License (DL) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]
- Certified for Oasis Specialist-Clinical (COSC) [Preferred]
