Registered Nurse Case Manager Home Health
About the role
As a Home Health Registered Nurse (RN), you will make a significant impact by supporting patient recovery and enabling them to stay at home, spending quality time with their loved ones and engaging in activities they enjoy. You will empower patients and families to achieve their healthcare goals by educating them on disease management and case manage and provide clinical oversight to an interdisciplinary team.
Responsibilities
- Support patient recovery and ability to stay at home

- Case manage and provide clinical oversight to interdisciplinary team
Requirements
- RN license in the state you work
- Two years of experience as a registered nurse, home health experience preferred
- Current driver’s license
- Current CPR and BCLS certification
