Job Duties

  • Serves in an expanded role to assess patient needs, plan and provide nursing care and coordinate follow-up care for patients receiving treatment in an outpatient Hematology/Oncology setting.
  • Primarily responsible for care navigation throughout the continuum of care.
  • Key responsibilities include patient education, triage and care coordination.
  • Coordinates the treatment plan coordination for Hematology and Oncology patients through the continuum of care.
  • Provides ongoing patient care and education for patients in the adult Hem/Onc Department.
  • Serves as a resource person for patient and family regarding the perception of the diagnosis and medical plan of care for the management of disease as designated by the physician.
  • Serves as a patient liaison between the patient and the care team as they encounter issues/obstacles/changes to their plan of care.
  • Performs pre-visit RN triaging for new and existing patients to ensure that decisions can be made on the first visit and treatment planning for the Hematologic/Oncologic care can be initiated.

  • Performs and assists with triaging telephone calls concerning health issues and offers appropriate telephone management advice to patients as liaison to physician recommendations.
  • Communicates lab, radiology and other testing results to patients and via telephone and electronic medical record letter notification.

Qualifications

  • Graduate from Specialty Training Program-Nursing (Required)
  • Bachelor's Degree-Nursing (Preferred)
  • Minimum Of 2 Years-Nursing (Required)
  • Certification(s) and License(s): Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Basic Life Support Certification - Default Issuing Body; Chemotherapy and Biotherapy certification courses within 3 months of hire - Default Issuing Body; Oncoclogy Certified Nurse required after completion of required hours - Oncology Nursing Certification Corporation (ONCC)

Pay

$10,000 sign-on bonus for eligible RN candidates.

Schedule

This role is full-time, 40 hours weekly; 8:00 a.m.- 4:30 p.m.
