Registered Nurse Coordinator - Cancer Care

Role Overview: An RN Coordinator in Cancer Care provides comprehensive nursing support in an outpatient Hematology/Oncology setting, focusing on patient assessment, education, care planning, and follow-up coordination. Key Responsibilities Coordinate treatment plans across the continuum of care for Hematology and Oncology patients.Deliver ongoing patient care, education, and serve as a resource regarding diagnosis and treatment options.Act as a liaison between patients and the care team, addressing issues and facilitating communication.Perform pre-visit triage, assist with supportive care referrals, and document chemotherapy teaching sessions.Participate in performance improvement activities and assist with staff orientation.Communicate test results and provide patient instructions to ensure understanding and compliance.Manage telephone triage, offering advice and coordinating care based on patient needs. Qualifications & Skills Minimum of 2 years RN experience; Hematology/Oncology experience preferred.Competency in Chemotherapy and Biotherapy administration, maintaining ONS certification.Ability to provide patient education, coordinate care, and communicate effectively. Additional Details: The role is full-time, typically performed in a clinical environment, requiring adherence to policies and ongoing professional development. Educational requirements include graduation from a Specialty Training Program in Nursing, with a Bachelor's degree preferred.
