Fairview Health Services is searching for a hardworking RN to join our Cancer Clinic in Burnsville. This position is approved for 80 hours every two weeks, with a schedule of Monday - Friday, 8:00 am to 4:30 pm.
Responsibilities
- Assesses patient needs and identifies expected nursing outcomes.
- Collaborates with patient, family, and other health team members to develop a nursing plan as part of an integrated, comprehensive plan of care.
- Develops an individualized plan based on age, social and cultural considerations, condition, and other relevant needs.
- Evaluates care and patient response to interventions and expected outcomes.
- Collaborates and communicates effectively and respectfully with the patient, family, and members of the health care team within and across care settings to promote partnership-based health care.
- Demonstrates the ability to advance care by utilizing standards, guidelines, care pathways, and continuous improvement to enhance patient outcomes.
- Promotes a culture of evidence-based practice (EBP).
- Coordinates care for patients and families in collaboration with the multidisciplinary team across the continuum of care, with an emphasis on newly diagnosed patients.
- Synthesizes assessment data and resulting diagnosis, plans interventions, and expected outcomes to provide patient and family with necessary information.
- Serves as a clinical expert/resource to oncology patients, families, health care team, and community.
- Assists the patient in coordinating multiple specialist appointments and provides education for patients.
- Coordinates multidisciplinary team conferences.
- Communicates with referring health care providers regarding the comprehensive plan of care for patient and family.
- Works closely with physicians and departments to deliver coordinated care in a timely manner.
- Works with the multidisciplinary team to develop quality monitors to measure patients’ outcomes and program performance.
Requirements
- MN Registered Nurse (RN) License.
- Basic Life Support (American Heart Association or Red Cross).
- Minimum of 1 year working in oncology nursing.
Qualifications
- Bachelor’s Degree in Nursing or BAN (preferred).
- 1-3 years of experience (preferred).
- 1 year of care coordination (preferred).
